Contribute and Grow Garantisce l'efficacia e l'efficienza nella gestione e corretta esecuzione dei piani operativi attraverso la verifica degli stessi. Garantisce i rapporti con i Medici di famiglia, i Distretti Sanitari e gli operatori del territorio per l'assegnazione del paziente in collaborazione con l'area pianificazione. Gestisce le telefonate di tipo clinico/sanitario e gli applicativi delle ASL (ove presenti). Effettua attività su delega del Coordinatore (es. gestione/consegna materiali sanitari, archivio, formazione applicativi e strumenti di lavoro agli operatori nuovi inseriti, ecc.) Contribuisce alle fasi di gestione dei rapporti con gli operatori sanitari. Responsabilità Erogazione del servizio domiciliare: Garantire l'assegnazione agli operatori delle attività assistenziali; Garantire supporto telefonico ai pazienti, caregiver, ecc.; Garantire il corretto utilizzo dei materiali e dispositivi messi a disposizione da Medicasa agli operatori del territorio; Verificare il monitoraggio degli outcome clinici, dati sanitari, le relazioni periodiche, la documentazione, ecc. utilizzando Cartella Clinica informatizzata e altre applicazioni a disposizione; Centrale Operativa: Partecipare alle riunioni di Team organizzate in Centrale Operativa; Garantire la redazione e verifica delle relazioni sanitarie periodiche e la documentazione clinica allegata a cartella clinica informatizzata; Formazione: Garantire il costante aggiornamento professionale con l'acquisizione dei crediti ECM come previsto dalla normativa nazionale; Garantire la partecipazione ai corsi di formazione organizzati da Medicasa Are you a MATCH?
