Are you passionate about supporting people experiencing homelessness in Plymouth?We are looking for a dedicated Housing First Support Worker to join our team and play a vital role in supporting individuals experiencing homelessness across Plymouth . This role offers the opportunity to work directly with people facing complex challenges, helping them access stable housing and the support they need to rebuild their lives.Working within Plymouth’s homelessness services and in partnership with local agencies, you will deliver a Housing First approach , ensuring people are supported to move out of homelessness and sustain their tenancies long term. You will work in a compassionate, person-centred and trauma-informed way, putting people at the heart of everything you do.Key ResponsibilitiesHomelessness Support:Work directly with individuals experiencing homelessness in Plymouth, many of whom have complex needs, to assess their circumstances and support them into secure, stable housing.Case Management:Develop and deliver personalised support plans, coordinate housing and support services, and provide ongoing, flexible support to help individuals sustain their accommodation.Multi-Agency Working:Build strong relationships with Plymouth-based partners including mental health services, substance misuse teams, health providers, housing teams, and voluntary sector organisations to ensure coordinated support.Crisis Intervention:Provide practical and emotional support to individuals in crisis, using de-escalation techniques and safeguarding awareness to promote safety and stability.Advocacy:Advocate on behalf of individuals experiencing homelessness, ensuring their voices are heard and they can access housing, health, welfare, and community services.Recording & Monitoring:Maintain accurate and timely case notes, risk assessments, and support plans in line with service requirements.Community Integration:Support people to reconnect with their community in Plymouth, including accessing employment, training, healthcare, and wellbeing services.Why Join Us?Make a real difference to people experiencing homelessness in PlymouthBe part of a supportive, values-driven team committed to ending homelessnessWork within a Housing First model that prioritises dignity, choice, and long-term stabilityAccess training and professional development in trauma-informed practice, case management, and complex needs supportIf you are committed to supporting people out of homelessness and helping them build a stable future in Plymouth, we would love to hear from you.Apply today and be part of Plymouth’s Housing First response to homelessness.BCHA Safer Recruitment: Safeguarding children and adults is our first priority, and we will ensure we follow safer recruitment procedures.As an employer, we will do everything possible to prevent unsuitable people obtaining employment with BCHA.
