Right at Home provide exceptional in-home care and support for a range of clients in the Perth South-Eastern Suburbs " Our Mission is to improve the quality of life for those we serve". We are seeking Community Liaison Officer who is motivated, reliable, and passionate about aged care.
Delivering services under a range of funding sources including private pay clients, My Aged Care , the primary responsibility is to provide quality in home care and assistance to our fast-growing client base.
The successful applicant will be working with our care team, liaising with Clients, Family Members, Staff, and a wide range of Stakeholders to ensure services are provided in accordance with client's needs, goals and preferences.
Assessment, planning, and reviewing client plans as required, services are Client focused and Consumer directed at all times with quality outcomes achieved using a flexible, responsive, creative and collaborative approach.
In addition to client management, this role will manage inside and outside sales enquiries, develop and maintain new and existing referral sources.
QualificationsThe Community Liaison Officer will be responsible for:
Building strong connections with clients and their familiesService coordinationAssigning staff to clients based on staff qualifications and availability, as well as client preferencesOrganise support to clients with disabilities and monitoring and reporting on their progressReferring clients to agencies that can provide additional helpAssessing client needs and resources for health, welfare and other facilities and servicesResponding promptly to emails, enquiries and requests from clients, staff and managementMaintaining accurate records of incidents and feedback as requiredRe-organising work priorities to meet the needs of a fast-paced environmentSupporting all clients to meet their goalsConducting Client Care Assessments and Care PlanningSupervision and management of Care StaffDiscussing and promoting Right at Homes services with medical professions and referral sourcesReporting to head officeAbility to work in a fast-paced environmentPreferred SkillsExperienced in assessing and planning client needsSound understanding of the Aged Care and Disability systemsA strong client focus which is based on their goals, preferences and needsDemonstrated experience managing staffCurrent National Police CheckWorking with Children CheckFull Drivers LicenceAll candidates must have the right to work in AustraliaOrganisational, administrative and time management skills#J-18808-Ljbffr
