• Use clinical assessment skills to identify sponsors with special medical and educational needs.
• Maintain procedures to ensure timely identification of sponsors with special needs.
• Assess psychosocial functioning and needs of patients and their family members.
• Formulate and implement treatment plans.
• Ensure sponsors with special needs are identified in medical special needs data management systems.
• Provide clinical training to installation personnel and medical staff.
• Collaborate with Integrated Delivery System and other interagency forums.
• Provide information, referrals, and coordination as needed.

Education and Qualifications:

Graduate of associates (ADN) or baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education.

A BSN is preferred.
Active, valid, current, and unrestricted license to practice nursing as a registered nurse in any US state/jurisdiction is required.

Minimum of two years full-time experience in care coordination, case management or discharge planning experience in a medical setting within the last four years is preferred.

Current Basic Life Support certification is a job requirement.

Language Requirement:
Read, understand, speak, and write English fluently.
Must have or be able to pass a T.1 background check.
Must also have or be able to pass a National Agency Check.

Knowledgeable in medical privacy and confidentiality (Health Insurance Portability and Accountability Act); accreditation standards of Accreditation Association for Ambulatory Health Care and The Joint Commission.

Ability to input, extract and format data from the Armed Forces Health Longitudinal Technology and Composite Healthcare Computer System.

Skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.

Organization, problem-solving and communication skills to articulate medical requirements to patients, families/caregivers, medical and non-medical staff in a professional way.
