Our RN Case Manager role offers competitive pay, health and retirement benefits, mileage reimbursement, self-scheduling for weekly visits, and on-call rotation. The coverage area is divided into geographic teams, serving Kerr, Bandera, Kendall, Medina, Kimble, Edwards, Real, and Gillespie County.
As an RN Case Manager at Peterson Health, you will be responsible for managing a caseload of hospice patients, utilizing advanced assessment, teaching, and decision-making skills. You will evaluate patients for appropriateness of hospice care, develop plans of care in collaboration with healthcare providers, caregivers, and the Hospice Interdisciplinary Team. Your role involves follow-up evaluations to assess the effectiveness of care plans, monitor patient and caregiver responses, and provide education based on provider orders, departmental policies, regulatory guidelines, and best practices.
The successful candidate will work collaboratively with healthcare providers, patients, caregivers, and the Hospice Interdisciplinary Team to meet care goals, considering patient limitations and preferences in accordance with hospice philosophy.
Estimated Salary Range: $85,000 - $115,000 per annum, depending on experience.
Required Education and Skills:
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Nursing skills defined by generally accepted standards of practice
- Good interpersonal skills
Required Licensure/Registration/Certification:
- Current Texas State license as a Registered Nurse
- CPR certified
- Current Texas Driver's License and auto insurance
Required Experience:
- Two years' experience as a Registered Nurse with med/surg experience
