Job Summary

We are seeking an experienced Registered Nurse to join our team as a Nurse Care Specialist. As a Nurse Care Specialist, you will be responsible for providing high-quality, patient-centered care to our clients in their homes.

Key Responsibilities

  • Provide holistic care to clients in their homes, including nursing assessments, medication management, and wound care.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to clients and their families on health-related topics.
  • Conduct regular assessments to evaluate the effectiveness of care plans and make adjustments as needed.

Requirements

  • Current registration as a Registered Nurse with AHPRA.
  • 3+ years of experience in community nursing.
  • Excellent written and verbal communication skills.
  • Able to work autonomously and as part of a team.
  • High level of time management and organizational ability.

Preferred Qualifications

  • Satisfactory Australian National Police check.
  • Current or willing to obtain a First Aid and CPR certificate.
