Medical Case Manager for Aviano Air Base

Job Summary: We are seeking a highly skilled Medical Case Manager to join our team at Aviano Air Base, Italy. As a Special Needs Coordinator / Registered Nurse, you will be responsible for treating and/or supporting the treatment of patients at the 31st Medical Group, Aviano Medical Treatment Facility (MTF).
About the Role: This is a full-time position that requires a graduate degree in nursing from an accredited program. A BSN is preferred. The ideal candidate will have a minimum of two years' experience in care coordination, case management or discharge planning in a medical setting. Active registration as a nurse in any US state/jurisdiction is mandatory. Current Basic Life Support (BLS) certification and fluency in English are also required.
Main Responsibilities:

  • Use clinical assessment skills to identify sponsors whose family members have special medical and educational needs.
  • Maintain procedures to ensure timely identification of special needs.
  • Collaborate with other key service providers to ensure effective outreach and identification of special needs.
  • Ensure family members of AF personnel are appropriately "Q coded" (EFMP-enrolled).

What We Offer: Competitive estimated salary €45,000 - €60,000 per year, depending on experience. Additional benefits include a T1 background check, National Agency Check (NAC), and expertise in medical privacy and confidentiality. Familiarity with Microsoft Office programs, MS Outlook, and Internet is essential. Ability to input, extract and format data from AHLTA and CHCS systems is also required.
