Make a Difference in the Lives of Children with Special Needs

At Phoenix Home Care & Hospice, we are dedicated to providing exceptional in-home nursing care to children and young adults with specialized health care needs. As an In Home Pediatric Private Duty Nurse, you will have the opportunity to work with children in their homes, creating a nurturing environment that brings joy and comfort to their lives.

Key Responsibilities:

  • Develop and maintain individualized care plans in collaboration with the healthcare team.
  • Administer prescribed medications and treatments as per plan of care.
  • Monitor and document patient status, and share updates with physicians and families.
  • Educate families on proper care and treatments to ensure continuity.
  • Maintain accurate and timely electronic health records.
