Job Summary

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will provide high-quality nursing care to patients in their homes, working collaboratively with physicians and other healthcare professionals to achieve optimal patient outcomes.

About the Role

The ideal candidate will have current RN licensure in the state of Florida, current CPR certification, and a reliable vehicle or access to public transportation. You will be responsible for providing clinical services within the scope of practice, making initial and comprehensive nursing evaluations, developing plans of care, and documenting accurate and complete information.

Responsibilities
  • Provides clinical services within the scope of practice, as defined by state laws governing nursing practice
  • Makes initial and comprehensive nursing evaluations, determines primary focus of care, develops plan of care, and submits timely documentation
  • Performs patient comprehensive assessments at designated time points, develops appropriate plan of care, and responds to outcome coordinator/coder and Patient Care Manager requests for clarification
  • Orients home health aides and licensed practical vocational nurses on tasks and procedures, ensures adequate Team Leader (TL) communication when physician follow-up is needed
Qualifications

To be successful in this role, you will need:

  • Current RN licensure in the state of Florida
  • Current CPR certification
  • Reliable vehicle or access to public transportation
  • Ability to communicate effectively with patients, families, and healthcare teams
