Job Summary

We are seeking a highly skilled Special Needs Coordinator to join our team at French Consulting. This role is responsible for providing exceptional support to military families with special needs, ensuring their medical and educational requirements are met.

About the Job

This is a full-time position, requiring a high level of expertise in care coordination, case management, and discharge planning. The ideal candidate will have a strong background in nursing, preferably with experience in a medical setting.

Responsibilities
  • Conduct thorough assessments of patients' needs and develop personalized treatment plans.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery.
  • Ensure all necessary documentation is completed accurately and efficiently.
  • Provide training and consultation to installation personnel on EFMP-M best practices.
  • Participate in quarterly case reviews to discuss newly identified families' needs.
Requirements

To be successful in this role, you must possess:

  • A bachelor's degree in nursing or a related field.
  • A valid, unrestricted license to practice nursing in any US state or jurisdiction.
  • At least two years of experience in care coordination, case management, or discharge planning.
  • Basic Life Support (BLS) certification.
  • Excellent communication and problem-solving skills.
  • The ability to work effectively in a fast-paced environment.
