Intake Coordinator

The Intake Coordinator will be responsible for performing and overseeing all resident intakes, initiating resident familiarity with program guidelines, and properly assessing an individual's qualifications before admission. This role requires close collaboration with the Intake Manager, Motivational Track Manager, and Shelter Managers to ensure a safe and appropriate transition for each new resident to their respective program.
Key Responsibilities:
- Ensure that intake is completed with Matthew 25: 35-45 principles
- Assess the appropriateness and eligibility of potential residents prior to admission
- Screen individuals on Shelter and Justin's Place waiting list
- Create and review resident records created at admission
- Document resident's medication in Mission Tracker
- Complete automated intake process through Mission Tracker
- Assign bed assignment along with resident chore and case manager
- Explain expectations of the program to residents at admission
- Conduct property search for each new intake
- Coordinate transportation needs for new residents
- Provide excellent customer service to internal and external customers
- Work with referral agencies to schedule admissions of those being discharged
- Check personal emails and MARS report daily and respond appropriately
- Participate in Team meetings and projects as assigned by Consolidated Intake Manager
- Presents the Gospel message to residents when the opportunity is available
- Pray with residents and staff as opportunities are available
- Complete all job duties with St. Matthew's House core values as the guiding principle
