Job Summary:
A qualified Licensed Practical/Vocational Nurse administers one-on-one skilled nursing care to patients on an hourly (shift care) basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.
Key Responsibilities:
- Implement nursing care plans for each patient, ensuring high-quality care and patient satisfaction.
- Provide nursing services, treatments, and diagnostic and preventive procedures as assigned, adhering to established policies and procedures.
- Initiate preventive and rehabilitative nursing procedures as appropriate, promoting patient independence and well-being.
- Monitor patient reactions to treatments, including medications and changes in physical or emotional condition, reporting to the physician and RN as necessary.
- Teach and counsel patients and their families/significant others regarding nursing care needs and other related problems, promoting patient education and empowerment.
- Prepare clinical and progress notes, maintaining accurate and detailed records of patient care.
Requirements:
- Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state of California.
- (1) year experience in nursing is a plus, new graduates welcome.
- Current CPR certification.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment.
