How will you CONTRIBUTE and GROW?
Reporting directly to the Digital Value-Based Coordinator, the Person will be responsible for contributing to the development of valuable services offered to patients undergoing treatment. This involves defining and delivering personalized support plans and their corresponding clinical follow-ups, with the goal of improving the therapy experience and ensuring optimal therapeutic adherence.
The Value-Based Case Manager will manage and monitor data from the remote monitoring platforms of devices used for respiratory therapies (C-PAP, pulmonary ventilators, etc.), aiming to improve therapeutic adherence in less compliant patients through personalized follow-up plans and educational phone support.
For the Marketing team, we are looking for a Value-Based Healthcare Case Manager:
Reporting directly to the Digital Value-Based Coordinator, the Person will be responsible for contributing to the development of valuable services offered to patients undergoing treatment. This involves defining and delivering personalized support plans and their corresponding clinical follow-ups, with the goal of improving the therapy experience and ensuring optimal therapeutic adherence.
The Value-Based Case Manager will manage and monitor data from the remote monitoring platforms of devices used for respiratory therapies (C-PAP, pulmonary ventilators, etc.), aiming to improve therapeutic adherence in less compliant patients through personalized follow-up plans and educational phone support.
Activities:
Manages and monitors data from digital platforms to ensure the implementation of personalized support plans for each patient.
Oversees the progress of digital therapies, collaborating with the Digital VBHC Coordinator, to ensure the correct definition of monitoring plans aimed at achieving the best therapeutic adherence.
Conducts personalized support programs (phone and remote) for patients undergoing remote monitoring.
Enters, monitors, and reports data from digital platforms into dedicated IT tools.
Develops and administers ad hoc educational reinforcement programs, in line with the personalized support plans provided for each patient.

Requirements:
Degree in scientific/healthcare fields: Nursing degree and/or Physiotherapy with a Neuro and/or Respiratory specialization.
A maximum of 3 years of experience in a similar role gained in structured environments, preferably operating in the Healthcare / Homecare, pharmaceutical, or medical device sectors.
Global mindset and international aspirations.

Leadership potential in approach.
Excellent interpersonal and communication skills.
Innovative and proactive thinker.
Fluent in English.
