Education

• Master's degree from an accredited school of social work
• LCSW – Licensed as a Clinical Social Worker

Experience / Qualifications

• Current licensure in the State of Florida, License Clinical Social Worker.
• 2 years acute care experience, SNF, or home health experience.
• Knowledge of community resources.  
• Ability to perform independently within scope of practice.
• Excellent verbal, written, communication and organizational skills. 
• Effective collaboration and participation as a team member.
• Effective interaction with physicians and healthcare team members. 
• Ability to set priorities, outcomes management, conflict management and data analysis.
• Must be skilled at using a computer, fax and copier.

Position Summary

The Licensed Clinical Social Worker is responsible for psychosocial assessments, therapeutic treatments, counseling and discharge planning, care coordination for patients and their families. 

• Identifies needed community services and coordinates patients' needs with the appropriate service. 
• Provide and assist the multidisciplinary team to help patient and family member's cope with patient's illness, diagnosis, treatment.
• Provides assistance and support to patients and families with life situations including emotional, mental and substance abuse disorders. 
• Negotiates care issues as appropriate in regard to indigent issues, resource availability. 
• Facilitate / participate in patient and family care conferences as needed. 
• Coordinate, plan and facilitate care transition upon discharge.
• Performs other duties as assigned.

Physical Requirements

Requires sitting for long periods of time, also stooping, bending, standing, and stretching. Must have manual dexterity for typing and computer data entry.  Able to work under a stressful environment, work independently, capable of making sound decisions, be detail oriented, alert, and self-motivated. Ability to push/pull up to 40lbs., carry and lift up to 20lbs

  • Annual Joint Commission mandatory education requirements, in-service and health requirements including attendance at new employee orientation
  • TB/PPD Surveillance Program
  • Maintenance of required professional licensing and/or certification(s).
