Intervention Support Services Coordinator Division : Disability Position No : Salary : Level 5, $105,167 - $114,938 pa + Super - PSCSAA 2024 Work Type : Fixed Term - Full Time, Permanent - Full Time Location : Como Closing Date : 4 : 00 PM (YYYY-MM-DD) About the Disability Division The Disability Division (the Division) develops and delivers policy, programs and services that enhance the inclusion and participation of people with disability in communities across Western Australia. As well as guiding the vision of an inclusive state, the Division is putting it into practice through a significant service delivery stream, including oversight of supported community living, intensive service support services and the disability justice program. The Role Intervention Support Services (ISS) provide through-care services for people with a complex disability requiring 24 / 7 transitional and / or emergency accommodation. Intervention Support Service Coordinators ensure the effective daily operations of an assigned ISS through leadership, guidance and support. More specific responsibilities include, but are not limited to : Providing early engagement and appropriate supports to assist individuals who require early intervention supports (this may be to the individual, to the sector and other Government organisations and / or to the families and carers). Promoting person-centred individualised support, fostering family relationships and encouraging community involvement and inclusion. Overseeing the accommodation to ensure appropriate facilities are established and maintained to meet individuals’ disability needs. Providing supervision and leadership to the Social Trainers and a Domestic. This includes supporting learning and development goals, responding to critical incident reviews, reflective practices and debriefing routines.
