TFI Family Services has the job for youWe are looking for a full-time Aftercare Case Manager to add to our team
All you need is a bachelor's degree in human services and a valid driver's license.
We will teach you the restA Day in The Life as a Case ManagerAs an Aftercare Case Manager, we provide support and services to children and families that have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth.
During this work with post-permanency placements the focus is on risk management for their designated aftercare period.
This can include crisis intervention for the child/family when necessary, working closely with the public and private agencies providing supportive services, maintaining accurate and timely documentation and reports, and coordinating community and educational services.
Aftercare Specialists meet with the child/family as determined by the aftercare plan, through in-person meetings to support the reunification and address or resolve concerns.What We Are Looking ForWe are looking for someone who is compassionate, hardworking, and devoted to the strength of families.
