About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager, RN to join our team at Care Centre. As a key member of our hospice care team, you will be responsible for overseeing the day-to-day operations of our hospice site, ensuring the provision of high-quality care to patients, and managing a team of healthcare professionals.

Key Responsibilities
  • Leadership and Management: Supervise, manage performance, and direct the work of team members, ensuring timely completion of visits and tasks.
  • Site Management: Manage the site in the absence of the Executive Director or Administrator, ensuring seamless operations and patient care.
  • Clinical and Administrative Functions: Oversee clinical and administrative recordkeeping, ensuring accuracy and compliance with regulatory guidelines.
  • Communication and Customer Service: Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
  • On-Call and Direct Patient Care: Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes, as needed.
Requirements
  • Education and Experience: Degree in nursing, Registered Nurse with ASN or BSN from an accredited school of nursing, one-year experience as an RN, and three years' experience in direct patient care, with one year of management experience.
  • Licenses and Certifications: Current license to practice nursing in the state of employment, CPR Certification, and valid driver's license.
  • Personal Qualities: Sensitivity to the feelings of terminally ill patients and their families, flexibility, effective communication, and ability to demonstrate patient advocacy.
