Mobile Health Team Inc. a staffing agency has openings for a Physical Therapist(PT) Home Care. We are looking for someone with a passion for people.

Location: Barre VT 05641

Assignment Start Date: 9/9/2024

20 weeks Days 8hour shift 40 hours per week with 36 hours guaranteed

Physical Therapists needed to provide oneonone therapy care for patients in their home setting

RESPONSIBILITIES

  • Assess a persons condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
  • Collaborate with interdisciplinary teams and colleagues to ensure a persons needs are met.
  • Demonstrate ability to perform standard therapy measurement techniques including ROM strength and balance tests.
  • Utilize a variety of therapeutic modalities to maximize the care provided to clients

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

  • Home Health experience and OASIS experience are a must as this candidate will casemanage
  • No RTO required

Licenses Certifications and/or Registration:

  • Vermont PT licensure is required
  • Basic Life Support certification is required

EMR System: McKesson/Netsmart EMR experience is strongly preferred

Shift: The first week will be MF to facilitate orientation. The position will move to SundayThursday for the remainder of the contract.
